This role includes specialized duties, to include but not limited to protective services, care transitions, and person-center counseling. Maintaining a caseload of consumers and involvement in necessary areas of assistance will be essential. You may have the opportunity to regularly coordinate services with physicians, community agencies, and financial institutions. • Full-time employment.
• Work hours are 8:00 am to 4:30 pm, Monday – Friday, with 60-minute lunch.
• On-call hours when required.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONSMinimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.
