This role includes specialized duties, to include but not limited to protective services, care transitions, and person-center counseling. Maintaining a caseload of consumers and involvement in necessary areas of assistance will be essential. You may have the opportunity to regularly coordinate services with physicians, community agencies, and financial institutions.
