If you have a passion for working with people, this is the position for you!DESCRIPTION OF WORKAs an Aging Care Manager 2, you will be working with older adults, their families, and professionals through ongoing care management responsibilities. This role includes specialized duties, to include but not limited to protective services, care transitions, and person-center counseling. Maintaining a caseload of consumers and involvement in necessary areas of assistance will be essential. You may have the opportunity to regularly coordinate services with physicians, community agencies, and financial institutions.   • Full-time employment.
  • Work hours are 8:00 am to 4:30 pm, Monday – Friday, with 60-minute lunch.
  • On-call hours when required.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONSMinimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.
