We are looking for a well experienced Certified Home Health Aide (CHHA) to work between the cities of Fontana to Redlands.Apply your home health nursing experience to provide high quality care to our patients. We thrive on our patient care, with user-friendly web-based documentation.Responsibilities and Duties:

• Performing Personal Care needs after given a patients assignment and attending to his/her Requests promptly.
• Meeting safety Needs of Patient and using equipment safely and properly.
• Giving personal care including baths ,Grooming, all around Hygiene care, and light housekeeping.
• Planning and preparing Nutritious Meals.
• Provide appropriate timely feedback to RN Case Manager and other professionals.
• Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency.
• Providing proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence.
• Reporting on patient’s condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
• Assisting in ambulation and exercise as instructed by the Registered nurse or MD/therapist.

Salary: $20 – 22 per hour
